Not every input becomes an output. The system is designed to say no — and to say it visibly.

How a case moves through the governed decision systemEvidence enters from four independent sources: system logs, customer signals, operational metrics, and human reports. These merge into two intake streams, each meeting an evidence-sufficiency gate. Cases failing either gate stop immediately at a logged termination marked insufficient evidence. Cases that pass enter AI analysis, from which any case exceeding the automated threshold exits to a separate termination marked human review required. The remaining flow enters human judgment, where a named reviewer confirms or rejects and silence is not treated as consent. Cases beyond that reviewer’s authority exit to a third termination marked approval required. Only the surviving thread continues through approval, action, and receipt, each recorded and attributable, before reaching a resolved outcome. Every terminated path is logged.
